Real-Time Care Improvement: Smart Clocks in Home Support

The CIUSSS de l’Ouest-de-l’Île-de-Montréal deployed 50 smart clocks in clients’ homes to improve autonomy, reduce avoidable visits, and support seamless communication with caregivers and family members. These clocks can be programmed with personalized reminders (e.g., medication, appointments, visits), reducing dependency on in-person care.

In just seven months, 6,365 home visits were avoided, saving over $80,000 and reducing 20,750 kg of CO₂ emissions. 64% of participants still use the device, with satisfaction rates above 90%. The project is grounded in a human-centered, technological, and inclusive approach — the interface is multilingual, and patients and families are actively involved. This easily replicable, innovative model shows how social innovation can transform home care for the benefit of users, caregivers, and the health system.

Plume AI Integration

Plume IA is a Quebec-based AI solution revolutionizing medical note-taking through automated clinical documentation. Created by two emergency physicians, it helps professionals save 1–2 hours per day by eliminating manual note-writing, while improving clarity and accuracy. Used by over 2,000 clinicians, Plume IA boosts clinical throughput (up to 6 more patients per day in some clinics), reduces stress, and has even delayed some retirements.

The app integrates seamlessly with existing clinical workflows, aligns with consultation pacing, and generates structured notes in real time. Already adopted by CHU de Québec, CHUS, and multiple GMFs, Plume IA is compatible with EMRs and improves interprofessional coordination. Intuitive, quick to implement, and designed with and for clinicians, Plume IA frees up time for care and enhances service quality.

CASEN Mobile Clinic

CASEN is an innovative model of specialized mobile services deployed in remote regions, based on a human-centered, interdisciplinary approach. With over 65 professionals in neuropsychology, speech therapy, occupational therapy, social work, and psychoeducation, CASEN works on-site to improve access, reduce clinical isolation, and provide coherent care pathways.

In four years, over 2,000 patients have been served across 1,500 km of territory, drastically reducing wait times and costs associated with travel to urban centers. This mobile model, co-built with communities, ensures effective connections between institutions, schools, families, and community organizations. CASEN offers a fast, coordinated response tailored to regional needs. It’s a concrete solution to the shortage of specialized services — improving patient experience and transforming rural healthcare.

Luci

Luci, an initiative by Lucilab, is a free digital platform that helps adults adopt healthy habits to prevent chronic diseases and reduce the risk of dementia. Accessible throughout Quebec, Luci offers interactive tools, simplified scientific content, and personalized support from health advisors. Users can assess their lifestyle, build an action plan, and progress at their own pace.

The results are compelling: 97% improved their knowledge, 92% took concrete action, and 75% increased their physical activity. Successfully deployed across several regions, Luci works with CISSS, GMFs, and community organizations to reach a diverse population. By combining innovation, accessibility, and scientific rigor, Luci has become a vital tool for health promotion in Quebec.
